Clinical Placement Form

Please note this form will take approx 35 minutes to complete(10 minutes to complete form, 25 minutes to complete Safeguarding). You will be asked to upload the below certificates from HSELand, please ensure you have these saved in advance.

1. Evidence of Professional Registration or Student ID number (which ever is applicable).
2. AMRIC Hand Hygiene Training, dated within the last 2 years
3. AMRIC Standard and Transmission Based Precautions, dated within the last 2 years
4. Fundamentals of GDPR Training -HSEland Certificate of Completion
5. Children First Training-HSELand certificate of completion, dated within the last 2 years
6. Safeguarding Questionnaire- Please watch the SFHD safeguarding podcast(Link Below)and complete questionnaire contained in the form, this will take approx. 25 minutes.
