Clinical Placement Form

Please note this form will take approx 35 minutes to complete(10 minutes to complete form, 25 minutes to complete Safeguarding) . You will be asked to upload the below documentation, please ensure you have these saved in advance.

1. Professional Registration or Student Registration Letter(which ever is applicable).

• Copy of Garda Vetting:
  – For applicants who are students in the Republic of Ireland, please request a copy of your Garda clearance certificate from your college
  – All other applicants must provide a copy of their Garda clearance certificate
• For applicants who are not students in the Republic of Ireland, please provide a copy of the equivalent police clearance certificate from your college or place of work. Please ensure it is translated if the document is not in English

1. AMRIC Hand Hygiene Training, dated within the last 2 years
2. AMRIC Standard and Transmission Based Precautions, dated within the last 2 years
3. Fundamentals of GDPR Training -HSEland Certificate of Completion
4. Children First Training-HSELand certificate of completion, dated within the last 2 years
5. Safeguarding Podcast & Questionnaire- Please watch the SFHD safeguarding podcast(Link Below)and complete questionnaire contained in the form, this will take approx. 25 minutes
6. If your first language is not English and if you have not been educated through the medium of English, you must present a proof of English language qualification .
