Clinical Placement Form

Please note this form will take approx 35 minutes to complete(10 minutes to complete form, 25 minutes to complete Safeguarding) . You will be asked to upload the below documentation, please ensure you have these saved in advance.

1. Evidence of Professional Registration.
2. Copy of Garda Vetting:
   – For applicants who are students in the Republic of Ireland, please request a copy of your Garda clearance certificate from your college
   – All other applicants must be Garda vetted by St. Francis Hospice Dublin. Please request the required forms from St. Francis Hospice Dublin.
3. AMRIC Hand Hygiene Training, dated within the last 2 years
4. AMRIC Standard and Transmission Based Precautions, dated within the last 2 years
5. Fundamentals of GDPR Training -HSEland Certificate of Completion
6. Children First Training-HSELand certificate of completion, dated within the last 2 years
7. Safeguarding Podcast & Questionnaire- Please watch the SFHD safeguarding podcast(Link Below)and complete questionnaire contained in the form, this will take approx. 25 minutes
8. If your first language is not English and if you have not been educated through the medium of English, you must present a proof of English language qualification .
